A scenario we see every week: someone brings in form 086/o, asks for a notarized English translation — and returns a fortnight later because the admissions office has sent its own form for a doctor to complete. The translation was correct. The document was not.

So let us start with the essential point: 086/o is a Ukrainian certificate for Ukrainian educational institutions. When it genuinely works abroad, and when it does not, is worth going through in order.

What 086/o is now, rather than ten years ago

Form No. 086/o was approved by Ministry of Health Order No. 110 of 14 February 2012. The version in force, however, is a different document: Order No. 1351 of 25 July 2023 revised both the form and its instructions and took effect on 21 November 2023.

What actually changed:

  • The title. It is now “Medical certificate (extract from the outpatient medical record)”. The older title, “medical consultative opinion”, survives only in old templates and on pages nobody has updated.
  • Who completes it. The certificate is issued by a primary-care physician — a family doctor, GP or paediatrician — from the data in your medical record. It is not a five-room commission.
  • Scope. It is the single form for enrolment in educational institutions at every level, from nursery to university.

For translation purposes one rule follows: the title in the translation must match what is actually printed on your form. If you hold the new version while the translation calls it a “medical consultative opinion”, you have created a discrepancy an official sees in a second.

086/o or 082/o: which certificate is actually being asked for

This is the most expensive confusion in the topic, because it surfaces after payment.

The rule is simple. If the requirement says “for travel abroad”, it means 082/o. If it says “for admission”, it means 086/o. And if it simply says “medical certificate” with no number, ask about the format before ordering anything.

What the foreign institution actually wants

This deserves candour, because it is where money is usually lost.

Universities abroad overwhelmingly have their own health form: a PDF sent with the admission pack, to be completed and signed by a physician. The Ukrainian 086/o does not replace that form — at most it is attached alongside.

Visa medical requirements are not met by translation. Where they exist, the receiving country names a specific accredited clinic and a specific examination, and the result goes straight to the authority. A Ukrainian certificate does not substitute for that procedure.

When translating 086/o genuinely makes sense: the requirement is worded generically (“a certificate of health”), or the document is for a sports club, a language course, a summer camp or an exchange programme, or a foreign school is enrolling your child and wants to see the Ukrainian medical history. In those cases a translated 086/o is a perfectly sufficient document.

The apostille: the MFA, and only if you really need it

Medical certificates are the category apostilled by the Ministry of Foreign Affairs — not the Ministry of Justice (which handles civil-registry documents) or the Ministry of Education (diplomas). Confusing them costs a trip to the wrong address. Our apostille route-finder answers it per document.

Where an apostille is needed, the sequence never changes: the apostille goes on the original in Ukraine first, then the translation is made — including the text of the stamp. The reverse order means paying for the translation twice.

Most admissions and visa processes, though, do not ask for it at all. Get the requirement in writing before you spend anything — the same principle that applies to every other document.

Validity — and why a certificate obtained “in advance” fails

Form 086/o carries no universal validity period of its own: the institution requesting it sets one. In practice medical certificates are treated as current for a few months, and some admissions offices require a document issued within the current academic year.

Hence the order of operations that saves both time and money: establish the filing deadline, then see the doctor, and only then order the translation. A certificate obtained six months early may turn out to be unusable — and a translation of an expired certificate is unusable with it.

A child abroad and a Ukrainian school: the certificate is not always needed

One situation comes up constantly: the family is abroad, the child also studies at a Ukrainian school, and the school asks for a certificate that cannot be obtained locally, because a foreign doctor will not complete a Ukrainian form.

That requirement changed on 1 August 2024. Ministry of Education and Science Order No. 714 removed the obligation to submit a medical certificate when enrolling in distance or individual forms of general secondary education: the head of the school may not demand it, and parents are not obliged to provide it. A copy is submitted only for full-time (day or evening) or blended enrolment. The ministry’s stated reason was direct: children abroad, in temporarily occupied territories and internally displaced cannot always attend a medical examination, and the requirement infringed their rights.

In practice, then, check the form of study before ordering a translation or travelling to Ukraine for a certificate. For distance learning the document is not required at all, in Ukrainian or in translation.

If the certificate is genuinely needed and you are abroad, two routes work: obtain it during a trip to Ukraine, or ask the school whether it will accept a local doctor’s medical opinion with a Ukrainian translation. The second depends on the school, so put the question in writing.

What matters in the translation itself

Medical translation is separate work, and not because of style. In form 086/o it is the small things that decide: diagnoses by ICD code, vaccine names, units of measurement in test results, the health group, clearance for physical activity. An error in units, or a smooth rendering of a diagnosis without its code, is what a foreign doctor reads as a contradiction.

The document is translated in full: every stamp, signature, the name of the institution, handwritten notes on the reverse. Names are spelled as in your international passport — check the official spelling with our transliteration tool.

The certification format depends on the recipient: an agency stamp is usually enough for a school, a course or a club, while notarial certification suits an official file. Formats and prices are set out on our medical certificate translation page.

Common mistakes

  1. Translating 086/o when 082/o was requested. The commonest and the costliest.
  2. Ordering a translation before the requirements are known. If the institution has its own form, no translation is needed at all.
  3. Using the old name of the form. The 2023 version is titled differently, and it shows.
  4. An apostille “just in case”. Admissions and visas rarely ask for it, and the MFA means a separate queue and separate costs.
  5. Translating only the front side. Entries, stamps and notes on the reverse are part of the document.
  6. Getting the certificate too early. The receiving institution sets the validity, and a head start works against you.

Frequently asked questions

What is the difference between forms 086/o and 082/o?

They are two different documents with different purposes. Form 086/o is the medical certificate for enrolment in an educational institution; the form was approved by Ministry of Health Order No. 110 of 14 February 2012 and its current version by Order No. 1351 of 25 July 2023. Form 082/o is the medical certificate on the state of health of Ukrainian citizens travelling abroad, approved by Order No. 332 of 13 August 2001. If you are asked for a "certificate for travelling abroad", it is almost always 082/o, and translating 086/o will not solve the problem.

Has the name of form 086/o changed?

Yes, and it matters for the translation. In the 2023 version the form is titled "Medical certificate (extract from the outpatient medical record)", whereas the older title was "medical consultative opinion". Many websites and templates still use the old one. The translation must carry the title actually printed on your form: a mismatch between document and translation is spotted instantly.

Who issues form 086/o and which doctors do you have to see?

It is issued where you are registered for primary care: your family doctor, general practitioner or paediatrician completes it after an examination and on the basis of your medical record. The familiar queue of five specialists each adding a stamp belongs to earlier practice; the scope of examinations is now set by the primary-care physician and depends on age and health. How long it takes, and whether additional tests are needed, is a question for that doctor.

Will a foreign university ask for form 086/o specifically?

Almost never. Foreign institutions and consulates work with their own paperwork: the university sends its own health form, and visa medical requirements are met at a clinic accredited by the receiving country. The Ukrainian 086/o is submitted at most as supporting evidence — for instance when a "medical certificate" is requested with no format specified. So establish first whether the receiving body has its own form, and only then order a translation.

Who issues the apostille on a medical certificate?

The Ministry of Foreign Affairs of Ukraine. Medical documents fall into the category apostilled by the MFA, not by the Ministry of Justice (civil-registry documents) or the Ministry of Education (diplomas). Before paying, make sure the apostille is genuinely required: admissions and visas rarely ask for it, while employment and court procedures abroad do so more often.
