Notarized translation of a medical certificate

A medical certificate is the document most often translated “urgently, because the filing is tomorrow”. The rush almost always has the same cause: certificates have a short shelf life, and their legalisation route differs from that of every other document. A diploma is apostilled by the Ministry of Education, a birth certificate by the Ministry of Justice — and a medical certificate by the Ministry of Foreign Affairs, which is the slowest of the three procedures.

First, check the form number
“Medical certificate” in Ukraine is not one document but a dozen different forms. These are the ones most often translated:

Getting the form wrong costs more than the translation: if a university asks for 086/o and you bring 082/o, the document is not “close enough” — it does not qualify. Check the form number against the receiving institution’s list before ordering. Medical certificates are apostilled by the MFA — and that takes longer
The division of competence in Ukraine runs like this: educational documents are apostilled by the Ministry of Education; documents of justice bodies, courts and the civil registry by the Ministry of Justice; and the remaining official documents, health-care certificates among them, by the Ministry of Foreign Affairs. The practical difference is not the name of the ministry but the procedure: to verify the signature and seal, the MFA sends an enquiry to the institution that issued the certificate. That step adds time, and it — not the translation — usually determines the overall timeline. So the working order is:
- obtain the original certificate from the institution;
- submit it to the MFA for apostille straight away;
- once it comes back stamped — translate it together with the apostille;
- certify the translation, with the agency stamp or notarially.

Check separately whether your institution’s certificate qualifies: requirements can differ for documents from private clinics and from state institutions, and that is a question for the apostille authority, not for a translation agency.
Validity period — the main reason files get spoiled
A medical certificate does not “last forever” the way a birth certificate does: most institutions want a document issued within the last few months, and some within the last 30 days. Hence the order of operations:
- first find out what age of document the institution accepts;
- then obtain the original;
- and only then order the apostille and the translation.
The reverse order is the classic mistake: a client translates last year’s certificate, and at the counter it turns out a fresh one is needed — so the whole chain starts again.
Why medical translation is a separate job
Three things in a medical text tolerate no approximation:
- Diagnoses and codes. Diagnostic wording and classification codes are rendered with the exact equivalent, never described “in other words”.
- Doses, units, values. The figure, the unit and the reference range carry over without rounding and without conversion, unless conversion was specifically requested.
- Handwritten fragments. Certificates are filled in by hand and seals are faint. We decipher everything legible, query what is doubtful, and mark “illegible” where reading is genuinely impossible — guesswork has no place in a medical translation.

Agency stamp or notary
For a kindergarten, a school or an employer abroad, certification with the agency stamp is usually enough — UAH 100 per document. Notarization (UAH 400) is needed where the certificate is part of an official file: immigration procedures, legalisation, some universities and insurers. If the requirement simply says “certified translation”, agency certification is in most cases the right fit; the procedure is detailed on our notarized translation page.

Frequently asked questions
Which certificate do I need to travel abroad — 082/o or 086/o?
They are different documents. Form 082/o is the health certificate for citizens travelling abroad: its form and issuing procedure were approved by Ministry of Health Order No. 332 of 13 August 2001, and it is the one embassies and employers ask for. Form 086/o is a medical consultative opinion, traditionally issued for admission to an educational institution or for employment. Before ordering a translation, check the form number against the receiving institution's list: translating the wrong certificate does not make it the right one.
Who apostilles a Ukrainian medical certificate?
The Ministry of Foreign Affairs of Ukraine. Not the Ministry of Health and not the Ministry of Justice: documents issued by health-care institutions fall into the category the MFA apostilles — unlike diplomas (Ministry of Education) and civil registry and court documents (Ministry of Justice). The particularity is that, to verify the signature and seal, the MFA sends an enquiry to the institution that issued the certificate, which can make the timeline noticeably longer than standard. The certificate is handwritten and the seal is illegible. What now?
That is an ordinary working situation with medical documents. We decipher what is legible and ask for clarification on what is doubtful — from you or, with your consent, from the institution. Where a fragment genuinely cannot be read, the translation carries an "illegible" note rather than a guess: an invented diagnosis or date is more dangerous than an honest annotation.
